It is due to the testamentary will of Bernardo Militari – archpriest of the Matrix of Erice – the foundation of this church and its Carmelite convent in 1423. The church overlooks the Piazza del Carmine, adjacent to the Casa Militari, home of its founder.
On its façade there is a portal delimited by jambs, surmounted by an entablature on which stands a window with a round arch; the plan is a single nave with chapels on its sides.
The Gothic style predominates, although the side chapels present heterogeneous styles due to the prolonged use of this building of worship.
Inside this church there was the sculptural complex of the Annunciation (1525) by Antonello Gagini, which is kept at the Polo Museale “Antonio Cordici”.
